2d) Cabinet underneath bathroom sink was unlocked and had plastic bags accessible to children 2f) Provider has two sliding glass doors and one of the glass doors does not have stickers/artwork at child's level 3a) Plywood covering sides of outdoor patio has splintered edges and remaining part of tree that had been partially cut down has sharp edges and is in licensed outdoor area accessible to children 3c) Sliding closet doors in room down hall are capable of pinching 3e) Multiple divots in backyard space and hose in backyard pose tripping hazards 3g) Two rusted chains were on ground in licensed outdoor space accessible to children 5a) Outlets in all licensed space are not Tamper Resistant and outlet outside above patio is also not Tamper Resistant . 5b) Outlets on either side of sink in kitchen and outlet next to bathroom sink are not GFCI or Tamper Resistant
